Note: Before choosing a hospital, refer to the list of Network Hospital &  Blacklisted Hospitals. We will be unable to pay claims-both cashless and reimbursement, for treatment availed for at blacklisted hospitals.​​​​​​​ We prioritise and expedite claims for senior citizens ensuring a smooth and hassle free claims experience.

Claim form- duly filled& signed (Required only for offline submission)
Personalized Cancelled Cheque with Proposer name on the Cheque / Passbook / NEFT form sign by Bank. In case of Death Cancelled cheque of Nominee
CKYC form with CKYC number mandatory for all claims. (PAN & Aadhar card of Proposer both are mandatory as per IRDA Guideline)
Discharge summary/Day care Summary.
Final hospital bill with supporting paid receipt.
Diagnostic reports (X-Ray, CT Scan, MRI Reports/Films)
Detail bill breakup (If Any)
Implant invoice/sticker (✔️ In case of Surgery)
Death summary (✔️ In case of Death)
FIR/Medico Legal Certificate (✔️ In case of accident)
Operation Theatre notes (✔️ In case of Surgery)
Pharmacy/Lab/Doctor Consultation bills with supporting paid receipt
Diagnostic reports/consultation papers as per attached pre-post bills and receipts
Medical prescriptions as per medical invoices
